How to make a referral
We want you to work with us by referring appropriate individuals to the service. You’ll be helping us to create a healthier, happier Hertfordshire, where residents’ physical and mental wellbeing are looked after.
Which patients are eligible?
Free support provided by Active Herts is available to patients/residents who meet the following essential eligibility criteria:
- Are currently inactive and do less than 30mins of physical activity per week
- Over the age of 16
- Live in the boroughs of Broxbourne, Hertsmere, Stevenage or Watford.
Additional inclusion criteria
Patients may also have any of the following:
- Diabetes Mellitus
- Hypertension
- Obesity (BMI >30, or BMI >28 if one or more co-morbidities)
- Smoker
- High Cholesterol
- History of heart disease
- Family history of heart disease
- Mild to moderate mental health condition
Exclusion criteria
The list below are contraindications to exercise. If patients/clients have any of the following they cannot be referred:
- Established ischaemic heart disease
- Hypertension Systolic: =>180mmHg or Diastolic: >100mmHg
- Uncontrolled Type 1 diabetes
- Severe or poorly controlled asthma
- Chronic pulmonary disease
- Cerebro-vascular disease
- Peripheral vascular disease
- Unstable or severe mental health state
- Tachycardia: > HR =>100BPM
- Febrile illness
- Heart failure
- Aortic Value Stenosis
